Seniors Outreach Services

The 411 Seniors Centre, in partnership with Vancouver Coastal Health , offers its I&R services to home bound seniors and seniors in care facilities throughout the Lower Mainland.This Project is funded by the City of Vancouver.Eligible clients are seniors over the age of 55 years and people with a disability who have barriers to leaving their homes.

What is the role of the Seniors Outreach Counsellor?

  • Helping seniors complete application forms for benefits such as Canada Pension, Old Age Security, and Guaranteed Income Supplement.
  • Providing information on various seniors’ services such as peer counselling, health services and community centres.
  • Referring seniors to appropriate community services such as advocacy groups around issues like poverty, elder abuse and legal services.

What is the process?

  • The service is accessed through the 411 Seniors Centre by phone at 604-684-8171.   The calls are routed to an Information and Referral counsellor, who makes the referral to the Seniors Outreach Counsellor who may live in the client’s neighbourhood or who may have specific skills that are needed.   Depending on the nature of the request, more than one visit may be necessary.

Who is eligible?

  • People over the age of 55 or people who have a disability and who have barriers to leaving their homes.

What special services are available?

  • Our counsellors speak a variety of languages and are non-judgmental and open to differences and diversity.   All requests are treated with confidentiality.   Counsellors are volunteers who have been screened and interviewed and who have taken extensive training in information and referral issues.

Who can make a referral?

  • The client, family, friend and health care professional can make a referral with the permission of the client.

What is the cost?

  • This is a FREE service.
